Gary is a recently diagnosed type 2 diabetic who is in need of education and support. Gary doesn’t know much about type 2 diabetes and doesn’t really have a clue about the best way that he can manage it. Gary’s insurance company and his doctor know that it is very important that he learns how to manage his diabetes so that he doesn’t get sicker and progress to type 1. They recommend Gary use Clevertar Digital Coaches which has a virtual health coach who educates him about his condition through a variety of interactive modules. After interacting with his coach for a few weeks Gary realised that his behaviour had changed as a result of his coaching and that he had made changes to his diet and exercise regime.
Mild to moderate anxiety and depression
Hannah has recently seen her GP and was diagnosed with mild anxiety. Hannah’s GP feels she would benefit from a lower intensity service and directs her to Clevertar’s anxiety and depression coach. The coaching content has been developed by a highly specialised team focussed on behavioural sciences and uses a low intensity cognitive behaviour therapy framework.
The coach uses validated measures to learn more about Hannah and is able to place her into a coaching stream that is specific to her needs. Hannah interacts with her digital coach using her tablet on a regular basis and uses the breaks between sessions to implement the strategies that she has learnt about.
Robert has just had heart surgery and has been transferred to the rehabilitation wing of his hospital while he recovers. Robert will stay here for around 3 weeks and in this time he uses Anna Cares as a way of reminding him to do his various exercises.
Anna also reminds him to take his medication and lets him know when his appointments are for his various specialists.
Robert’s nursing staff gain invaluable insights into Robert’s health as Anna monitors his daily well being and reports back to them via the web portal.
Clevertar Digital Coaches
Clevertar digital coaches are smartphone or tablet avatars – also known as ‘relational agents’ – that are designed to improve the health and wellbeing of patients by positively influencing their behaviour.
Digital coaches act like a ‘real’ health coach – they provide interactive education about the patient’s condition, help patients set goals about their self-management, and monitor and follow-up on progress, so that new behaviours are maintained. At the same time, Clevertar’s digital coaching platform gives providers an insight into the success and value of the coaching program for patients and populations via real-time reporting.
Clevertar’s digital coaches are an innovation in population healthcare, influencing behaviour and encouraging patient engagement at a fraction of the cost of human intervention.
Measurement and Monitoring
Our digital coaches are able to monitor a patient’s health through regular surveys and deliver a tailored coaching solution to suit their needs
Clevertar’s flexible digital coaching platform gives providers the ability to deliver custom health coaching content to their patients. Our coaches educate and interact with patients in a way that is powerful, engaging, cost-effective and easily accessible.
Software from the Cloud
The Clevertar Digital Coaches platform offers an app for patients and a web portal for clinicians to allow them to monitor patient progress. Commissioners of services can also access the web portal to evaluate program benefits at a population level.
